Insurance
I am an out-of-network medical provider. I do not accept payment from any insurance plans due to the nature of the services I provide and the amount of time I spend with each patient, which is significantly different than in conventional psychiatric or medical practices. Most insurance plans offer out-of-network benefits. I do not submit claims to your insurance. Upon request, I will provide you with a receipt that contains your diagnosis, treatments, and costs. You submit this receipt to your insurance company, along with an out-of-network reimbursement form that you can obtain from them. The insurance company will subsequently send you a check reimbursing you for all or a portion of your appointment. Please contact your provider to verify how your plan compensates you for mental health services.
I recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include out-of-network mental health benefits?
- When does the coverage end or begin?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- In what time period must claims be filed?
- Do I need written approval from my primary care physician in order for services to be covered?
Financial Policy
Payment is due at time of service. I accept cash, check, and most major credit cards as forms of payment. Please include the full name of the patient if paying by check. Any returned checks will be subject to any bank charges incurred by Jolie Carter Diaz, MD, LLC. A valid credit card number will also be requested. It will be charged at the end of the session if another form of payment is not provided or if you miss an appointment without providing 24-hour notice. Fees are subject to change annually.
Cancellation Policy
Appointments are scheduled according to each patient’s needs and my availability. The scheduled time is reserved specifically for you. Treatment is most effective when appointment times are regular and consistent. If you are unable to attend an appointment, please make sure you cancel by leaving us a voicemail message at least 24 hours prior to the appointment. Except in the case of emergencies, missed appointments that were not cancelled in advance will be subject to a cancellation fee. If you arrive late to your appointment, you will still be responsible for the full fee but your appointment time may be cut short. Please note, insurance does not cover the charge for missed appointments and these charges are your responsibility.
